How to Book

By Phone: Call the practice to speak with a receptionist who can assist with booking.

 

Be Prepared: When booking, be ready to explain the reason for your consultation. We will require your medical history to enable us to make informed evidence-based decisions for your treatment. Please ensure you complete the medical history form that is forwarded to you, and/or provide a printout of your NHS App summary and/or ask your practice to print your medical history summary to bring along with you so that we can add to your private confidential medical records. For longer appointments, specify the additional time required.
