Key Information Before Booking

A full copy of your medical records is required before an appointment can be booked (summary records are not sufficient).

Payment is required upfront and covers both parts of the assessment.

A completed fostering medical assessment form must be provided in advance.

The assessment is made up of two parts: GP administrative review and a face-to-face appointment.

Appointments are not available to book online and must be arranged by phone or email.

We will provide confirmation to your social worker or fostering agency once the assessment is complete.

Consent must be provided to allow PGPL to work with third parties.
